What to wear:
You will need to wear comfortable clothing that you can move in for the exam.
Absolutely no skirts or dresses ladies, and no super thick shorts or jeans.
ID and Insuarance Info:
Please text the front of your picture ID and the front and back of your
insurance card to 340.227.6409 as soon as possible.
LENGTH OF APPOINTMENT:
Plan 1.5 hours from the beginning of your appointment until you leave.
CANCELLATION FEE:
Please change your appt with 24 hour notice or there will be a fee.
PARKING:
behind the building – 2 “Serafina Guest” Spaces, to the right of the dumpster or #208. If
those parking spaces aren’t available then continue on frontage road towards Windward Passage
hotel, there are several parking lots along there.
LOCATION: above the Asfour Dept Store between Moe’s Market and First Bank.
Entry to office is through the glass doors facing the water between the Asfour Windows. You will see a Yellow “Chinnery Plaza” directory and a keypad. Please Dial 205 on the keypad to buzz into our office.
Only park in spaces outlined above in back lot otherwise you will be towed
